Psychiatric chart and record review
Focused psychiatric review of an examinee’s medical and treatment records, performed standalone or as a precursor to an in-person examination.
When chart review is used
- Pre-IME triage — before authorizing the cost of a full IME, counsel or adjusters can have the existing record reviewed to assess what the records show and what specific questions the IME would need to address.
- Standalone expert opinion — in some litigation contexts a record-only opinion is sufficient (e.g., rebuttal of an opposing expert’s methodology, opinion on treating-provider records, or where in-person examination is infeasible).
- Second opinion on an existing IME — review of an opposing or prior expert’s report and underlying records with critique of methodology, diagnostic conclusions, and causation analysis.
- Case validity assessment — preliminary opinion on the strength of the psychiatric component of a claim before significant case investment.
- Deposition preparation — chart review focused on the issues likely to arise in deposition of an opposing expert.
- Daubert / Sargon analysis — methodology review of an opposing expert’s opinion for admissibility challenges.

- Diagnostic imaging reports (MRI, CT, EEG) — with imaging review where relevant to TBI cases
- Neuropsychological testing results
- Pharmacy / medication records
- Deposition transcripts of treating providers and other experts
- Workers’ compensation claim file materials
Output
Written opinion structured to address the specific questions counsel or the adjuster needs answered — typically diagnosis, causation, apportionment, MMI status, treatment recommendations, and (where relevant) impairment-rating preview. Reports are deposition-ready and citation-supported where the opinion warrants.
Turnaround
Standalone chart review reports are typically delivered within two to three weeks of receipt of complete records. Expedited turnaround is available for time-sensitive matters — please indicate the deadline at intake.
