Dr. Cichon holds active medical licensure in five states. Nevada is the
primary in-person market and California is the primary growth market for
civil litigation and personal injury work. Utah, Arizona, and Wyoming
are served via telemedicine on a case-by-case basis.

In-person · full service catalog
Las Vegas in-person practice. Member of the Nevada Panel of Treating
Physicians and Chiropractors (NRS 616C.090). NIRSAT 86% —
Nevada Impairment Rating Panel certified. ABIME 6th-Edition and
AAEME 5th-Edition Guides certified. Subspecialty board-certified
TBI psychiatrist (one of two in NV). Workers’ comp, personal
injury, expert witness, Rule 35, and SCS clearance work.

Civil & PI only · telemedicine + in-person on volume
Active California medical license #196699 (since 2024). Telemedicine
examinations and expert witness work for personal injury, civil
litigation, and defense matters. In-person travel to California
available for substantial matters. Not a California QME
— we do not perform California workers’ compensation IMEs.

Telemedicine coverage
Active medical licensure with telemedicine availability for occasional
cases. We do not actively pursue volume in these states, but we do take
cases for established referrers and for matters that align with our
TBI / PTSD subspecialty focus.

Active Arizona medical license #66616 (since 2022). Arizona statute
(A.R.S. § 23-1026) explicitly authorizes telehealth IMEs with mutual
consent of both parties — one of the most telemedicine-friendly
IME jurisdictions. Carrier and self-insured employer engagements;
personal injury and civil work.

Telemedicine · tri-state consultation
Active Wyoming medical license #14893A (since 2022). Consulting
psychiatrist for Tri-State Medical Legal on Wyoming Workers’
Compensation matters. Wyoming statute (Wyo. Stat. §
27-14-102(a)(xi)(J)) requires WC mental injury diagnoses to come
from a licensed psychiatrist or clinical psychologist.
